Asthma
Asthma Reviews
If you have been diagnosed with asthma you will be invited for an annual review with the Practice Nurse. During these reviews the Practice Nurse will discuss your asthma control, check your inhaler technique and optimise your treatment. This appointment will typically take 20 minutes.
When you are due for your review you could complete an online review using the form bellow before your appointment. You can also check your inhaler technique using these videos.

Asthma Action Plans
We encourage patients who have asthma to have a written asthma action plan as this makes you four times less likely to be admitted to hospital for your asthma. You can download the action plans below and the Practice Nurse can help you fill this in.

Sustainability in healthcare is a very important priority and at Bron-Y-Garn we are looking at ways in which we can help improve the environment. Recycling your inhalers by taking them to our local pharmacies who support the Complete The Cycle project is a very easy change to make. A large proportion of the NHS carbon footprint is due to a certain type of inhaler, the Metered Dose Inhaler. Simply changing this type of inhaler, where appropriate, to a Dry Powder Inhaler can have a significant environmental benefit.

Baby Clinic
Practice nurses with the doctors and health visitors do the infant development clinics and appointments are not necessary.
The clinic provides developmental screening, immunisations and general advice (for well babies only please).
This clinc is held every Thursday afternoon 1:30pm to 2:30pm.

Breast Screening
Breast screening is operated and arranged by Breast Test Wales.
Cervical Screening: Smear Tests
Women aged between 24 and 64 should have a cervical screening every 3 to 5 years to help prevent cervical cancer. The screening is quick and painless and can be done here in the practice.
If you are aged over 24 and have never had a smear test, or if it has been more than 3 to 5 years since your last screening, you should arrange an appointment with our Practice Nurse. You should not have the test while you are having a period or in the 4 days before or after your period as this can affect the sample.
Contraception
There are lots of methods of contraception to choose from. Our nurses are able to carry out pill reviews. If you are wanting to start something new please speak to one of our doctors. If you are due for a contraception review this can be done by making an appointment with the Practice Nurse.

Coils
The Mirena Coil can be used for contraception which lasts for 8 years (previously this was 5 years but guidance updated in January 2024). If you had your coil fitted for contraception cover prior to this it will now cover you for 8 years from the date of fitting.
The Mirena Coil can be used to help manage heavy or painful bleeding or can be used as part of Hormone Replacement Therapy(HRT). It will last for up to 5 years for these reasons.
The Copper Coil can be used for contraception which lasts for 10 years. It can also be used for Emergency Contraception. Please contact the Sexual Health Clinic for further information on Emergency Contraception or for any further Sexual Health Advice.
COPD
Asthma Reviews
If you have been diagnosed with Chronic Obstructive Pulmonary Disease (COPD) you will be invited to an annual review with the Practice Nurse. During these reviews the Practice Nurse will discuss your symptoms, check your inhaler technique and optimise your treatment. Spirometry is often performed during this appointment, this appointment typically takes 20 minutes.

Pulmonary rehabilitation is a specialised programme of exercise and education designed to help people with COPD. It is the single thing that makes the biggest difference to quality of life for people who have COPD. If you would like to explore this further please speak with the Practice Nurse during your appointment.

Diabetes
Annual Diabetic Check
Our diabetic patients are invited to an annual diabetic check with the practice nurse which involves having your bloods and urine checked, a blood pressure and a foot check.
There are two different types of Diabetes, Type 1 and Type 2.
Diabetic Eye Screening
In addition to your diabetic annual check at the practice you will also be invited for a diabetic eye screening appointment in the community each year. Learn more about diabetic eye screening.
Self-Help and Self-Referrals
First Contact Physiotherapy
Who is it for?
The First Contact physiotherapy service is for people aged 16 and over with musculoskeletal conditions. Musculoskeletal conditions affect bones, muscles and joints.
You can request an appointment with a physiotherapist at Bron-Y-Garn Surgery. If you book an appointment with a First Contact Physiotherapist, you do not need to see a GP first.
Physiotherapists are experts in musculoskeletal conditions. They can:
- assess your problem,
- diagnose the issue
- decide with you about a suitable management plan to help return to things that are important to you.
What do I need to know?
Many musculoskeletal (MSK) conditions do not need treatment and can improve with what is called ‘self-management’ which describes the things that you can do to help yourself. If you want to know more about this you can follow this link. This approach will be discussed with you in your appointment if appropriate and the Physiotherapist can advise you on this.
What happens next?
To book an appointment with a physiotherapist for an assessment, give us a call and ask for an appointment with the physiotherapist.
If there are no available appointments, you can still refer yourself to physiotherapy. Your referral will be reviewed and added to the waiting list. If needed, a physiotherapist may contact you by phone to clarify the information you have provided.

Long Term Condition Care
An integrated system of care is offered to patients with long term conditions such as asthma, diabetes and heart disease.
We are now changing the way that we undertake your annual review for your Long-Term Condition(s). We are aiming to complete your annual review during your birth month, this will make it easier for us all to remember and prompt when you are due to be seen.
If you suffer with more than one Long Term Condition, we would like to complete all the relevant reviews during one visit. You may have had your annual review a few months ago but we would like to see you again to re-set your annual reviews to align with your birth month.
Patients are encouraged to set up reminders within their birth month and are welcome to book for their annual review in advance.
You will be invited by text / phone call or a letter to book your appointment.
For a lot of the Long-Term Condition Reviews excluding Asthma & COPD, you will require a blood test and will need to provide a urine sample. If you could bring a urine sample along with you to your appointment, preferably complete on the day. You can pick up urine sample pots from the surgery prior to your appointment.
Dependant on the outcome of results for your initial Annual Review, you may not be required to see a doctor. If it is decided that a doctor’s appointment is required, then the Surgery will contact you to arrange.
If you suffer with Asthma or COPD you will need to bring your inhalers with you to the appointment.
Minor Surgery
At Bron-y-Garn Surgery we offer a minor surgery service for non-cancerous skin lesions. This service is for non-cosmetic indications. Book an appointment with a GP who will discuss this further with you.
