New Patient Registration
Register with the practice as a new patient
How to Register
To join the practice, you will need to collect registration forms from our reception area.
All new patients will be given an appointment for a new patient check with the Practice Nurse.
Please provide a copy of your latest repeat medication list so that these can be added onto our system.
If you have come from a different health board, we may not be able to presribe all your regular medications as they may be hospital only in Cwm Taf Morgannwg Health Board.
Please note
Only patients within the practice boundary area will be registered.
Check you live within the boundary area
To be eligible to register at the practice, you must live within the practice catchment area. If you have any questions about your eligibilty join this surgery please contact us.
